Appropriate CASSP Referrals
•    To coordinate multiple agencies working with the child
•    Having difficulty accessing appropriate services
•    To explore various options in preparation for court hearing
•    To plan transitions
•    Multiple children in home accessing services from multiple providers
•    Unsure how to access funding for services

Inappropriate CASSP Referrals
•    No contact has been made with any programs
•    Emergency services
•    "Dead-set" on accessing a particular program/service

Okay, I have an appropriate referral, now what do I do?
•    Review the Parent's Guide to CASSP with the family
•    Complete a CASSP Release of Information
•    Complete the one page CASSP Referral
•    Fax/mail/email the CASSP Referral to the CASSP Coordinators
•    Fax/mail the Release of Information to the CASSP Coordinators
•    A CASSP Coordinator will contact the referral source to start the process
